Important point missing here: It’s not just the potential deaths (and very serious side effects). The drugs’s phase III clinical trial failed to demonstrate a benefit as measured by the study’s primary endpoint. In other words, their study did not demonstrate a benefit by their own standards. Secondly, to be accurate the FDA did not pull the drug from the market, they asked Sarepta to voluntarily withdraw it. Slight difference. One of Dr Prassad’s chief criticism and guiding philosophy is that many pharmaceuticals do not undergo the level of research that they should. This is at odds with the Trump “right to try” philosophy surrounding access to experimental drugs for terminally ill patients. This policy has some inherent ethical problems and I imagine Dr. Prassad is not fully a supporter. Surely Sarepta was frustrated their drug was pulled (technically they voluntary withdraw it at FDA’s request). This occurred after a 3rd patient died… who was later found to have died from unrelated causes- potentially part of the reason for reversal. It’s not just big pharma that was upset about the drug being withdrawn, there are many patients and families who are totally invested in having this drug work. There are no compatible alternatives and DMD is a terminal disease. I have long had my problems the FDA as a doctor. I disagree with some of Prassad’s opinions but I actually think he’s one of the more competent and principled MAHA appointees. Many of these people I am not impressed with and think they have the potential to cause more harm than good with their radical ideas. Just based off my instinct and limited data available for this drug, I’m thinking Dr. Prassad was on the right side of this. The FDA has become very lax in some respects… too many cases of ehhh it doesn’t really seem to work, it probably increases risk if stroke but we’ll allow it onto the market and then we can study it more… Prassad was against this philosophy as am I and most doctors.

I am a resident physician and I truly appreciate how Dr. Prasad approaches the evaluation of treatment and efficacy of new medications and guidelines. It is such a shame to see him removed from the FDA.
