This has been happening for a long time. Remember when Trump was holding rallies and he spoke, several times, about Hannibal Lector as if he was a real person? He definitely sounded like he thought he was talking about a real person. We are in real trouble when the President of the United States can't mentally distinguish between a real person and a movie character.

This is extremely dangerous. My dad had Parkinson’s disease dementia. In his last two months, he was seeing things that weren’t real, but real to him. Horrible things, like Snakes and wild animals in his room and the house being on fire. He was always the kindest and most reasonable person. On his better days, he would say, he was “living in a movie”. …What happens when Trump believes the threats that aren’t real, then what?! He has surrounded himself with ignorant, arrogant people who carry out every action this psycho demands. We are in even more danger with Trumps psychopathy.
