Decoding Insulin Resistance: The Hidden Core of Chronic Disease

Gain control over the root cause of most modern chronic diseases by understanding the two pathways to insulin resistance and the four pillars for reversal.

Short Summary

  • Insulin resistance (IR) is the common metabolic core for conditions like hypertension, Type 2 Diabetes, and Alzheimer’s disease, affecting an estimated 88% of US adults.
  • IR develops via two pathways: acute (reversible quickly via stress or inflammation) and chronic (driven by sustained high insulin and detrimental fat cell expansion).
  • Fat cell size, regulated primarily by insulin, dictates metabolic health, with visceral fat posing the greatest immediate threat compared to subcutaneous fat.
  • Longevity correlates strongly with insulin sensitivity and good glucose control; surprisingly, high cholesterol levels may also indicate longer life.
  • Combatting slow IR requires adherence to four pillars: controlling carbohydrates, prioritizing protein/fat, and implementing a structured fasting protocol.

This conversation with metabolic scientist Dr. Benjamin Bikman unpacks why insulin resistance is driving the epidemic of chronic illness and provides actionable metabolic strategies for reversal. Learn how environmental factors, common medications, and dietary choices dictate your cells’ responsiveness to insulin, shifting the treatment focus from managing symptoms to addressing the metabolic origin.

Why do I have to come to a podcast to learn more in a single episode than I have from my physician in the last 20 years.

I just wanna share my story. I was diagnosed with PCOS and was 89kg height 160cm in 2021. My gynae did say I was borderline diabetic but not yet. However, I was prescribed a diabetes medicine metformin but I decided there must be a more natural way than just eating medicine. I came across videos about fasting and keto and never looked back. I was slowly losing weight over a period of 7 months and down to 65kg. Got pregnant naturally eventho I’m scheduled for an IVF. Im now a mother of one to a healthy baby with no complications :)

I love how at times it looks like Dr. Bikman has finished his point, but then Steven stays silent and the doc keeps going deeper and adding color to his explanation. This style of interviewing makes these conversations so valuable.

I am morbidly obese. 68 days ago i was 280 pounds and prediabetic. In these 68 days, i cur out sugar and flour entirely and i reduced my calories to about 1800 daily. I have lost 10 kilos so far! I have a long way to go but i am very pleased to know that my high protein, high fat, low carb diet is the right path. And my blood sugar is normal now. 🎉

I just have to say that this guy’s scientific approach has helped me understand everything I needed to know about insulin. For the past three months, I’ve been exploring a ketogenic, high-fat carnivore diet—getting about 80–90% of my energy from animal fats, mostly butter. It’s become a hobby of mine, and I’ve consumed countless papers, videos, and podcasts on the topic. But when I came across him and this interview, something clicked: his clear, scientific explanations helped me truly internalize the logic behind keeping insulin low and following a very high-fat carnivore approach. I’m grateful for that.
