The main point of the safety rules is that you have to break more than one rule for a tragedy to happen…. If you always follow the rules this wouldn’t have happened…
I’m an IATSE grip and I’m not remotely surprised that this happened. What shocks me is the lack of safety meetings. I’ve attended safety meetings over a single lit candle on set.

Bottom Line: If you squeeze the trigger, you are responsible for the consequences. NEVER assume the gun is empty. It isn’t until you yourself check it. NEVER. Even then point it at someone, unless you actually want to kill them.NEVER squeeze the trigger with the gun pointed at someone, even if you are absolutely sure the gun is empty. NEVER treat a gun as if it were a toy. Respect it at all times.

Emily, at 23:51 I respectfully suggest that the unintentional discharges that occurred on the set were negligent discharges, not misfires (misfire is when gun doesn’t fire when you pull the trigger). Some people would use the term accidental discharge, but the term negligent discharge comes from the fact that if a gun goes off when it shouldn’t have, it’s almost always because you put your finger on the trigger when you shouldn’t have.
