Ngl, the fact that the wife trusted medical professionals so much is probably what helped break her delusion so quickly. She wouldn't trust OP or her parents saying that it was all in her head, because that's part of the delusion, but a psycho-therapist slowly saying "hey, this is all in your head" was more reliable to her because she trusted the therapist to know what they're talking about and begin helping her decstruct what her mind made her believe. I'm glad it resolved well. Last time I heard about Capgrass, a guy tried to murder his wife because he thought she was an imposter.

My first thought wasn’t cheating or schizophrenia but a tumor. Had no idea that there is an actual disorder that makes someone believe people in their life were replaced with identical imposters.

This is why I find the brain so fascinating *and* terrifying. The body can make a few simple adjustments between neurons that causes a spiral of mis-wiring that leads to delusions like this, or even full-blown schizophrenia. Like, there are a lot of people with schizophrenia that know when they're hallucinating, but the brain _still_ tries to convince them that the hallucinations are real.

My grandma was in the ICU for a week, and toward the end she 100 percent believed she was in a virtual world. The food was fake, the meds were fake, the people werent real. The only thing that was real to her was bottled water. She maintains even now, a month later, that it was all real and she isnt sure how they put her in a virtual world. The human mind is incredible and terrifying.

This... Is terrifying. This seems too quick to recover from. It's as though she's playing along, to try and find out what happened to her "real" family.. OP will worry about this forever.

The Capgras delusion is a neurological problem. To put it simply, there’s a part of your brain that activates when you see someone you love. In these patients, that part of their brain doesn’t work, which leads to a growing suspicion that their loved ones have been replaced with doppelgängers, since why would they feel so different when they look at them? Interestingly, there are documented cases where the delusion only holds visually, not auditorily. When they talk to their loved ones on the phone, for example, without seeing them, that suppressed part of their brain lights up again. In those cases, they’ll believe it’s their real loved one if they hear them on the phone but not when they see them in person. I’m no doctor, but maybe an MRI is in order?

Yep. Capgras syndrome was what came to my mind immediately. In a way, OP is lucky it was JUST that because the sufferer only thinks their close friends, family, and/or pets have been replaced, and not everyone else. That’s why she was willing to listen to the psychotherapist. Because that person was new to her. If her paranoia was more general (like thinking everyone is out to get you, not just people you’re close with), it could’ve been even harder to get her help.
